We accept all the indemnity or conventional plans of the companies below. We do not accept an HMO's or DMO's. If you have signed up for these, you have been assigned a dentist.
Please note that some plans have waiting periods, limitations, alternate benefits and missing tooth clauses that may limit your benefits. Consult the carrier, your HR person ro your benefits book. We are not supplied the detail your HR person or benefits book yield. You are responsible for obtaining these exclusions.
To our patients we are sorry to inform you that unlike previous years our office is no longer considered Prefered Providers for the following insurance companies: Guardian,Cigna, and GE Financial. If you are insured by one of these comapanies we appreciate your business and as always you are welcome to our office but, your visit and procedures will be paid at out of network costs.

Understanding Insurance
Insurance can be very confusing. We are happy to assist you in maximizing your benefits but your insurance is a contract between you, your employer and the carrier. We are a third party.
This is were the confusion may begin. Some patients believe that the filing, tracking, and collection for their insurance rests soley on the medical or dental office. The ADA, AMA, your HR person and even your carrier will confirm that this is your insurance and you are the responsible party. Despite this we will file your claim as a courtesy and use reasonable measures to track and follow-up. We also do pretreatments, check by phone and website to verify and gain information to assist you. We suggest that you also research this information. Your HR department is your best source of information as they deal with your policy and benefits.
OUR PROTOCAL AND POLICY
We ask that you provide your accurate insurance information at your initial visit. We will file your claim at our expense as a courtesy. We will try to obtain as much information about your plan as the carrier will allow. After 30 days have elapsed we will research and refile this claim. At 30-45 days, we will send you a letter informing you of the problem and ask that you become involved with your insurance company. At 60 days we ask that you satisfy the balance. We will refile or appeal without any expense to you, but direct the monies to you as you have met your obligation to us. In the unfortunate event, this account reaches 90 days. We will call or inform you of a final notice. On the 97th day we will reluctantly forward this to our agency for collection purposes. We are always happy to work together as your partner to solve these challenges that are becoming greater.
Financial Menu
A prepayment courtesy of 5% (10% if Senior Citizen, over 65) will be subtracted from the total patient obligation (not from any portion due from insurance company) if the patient obligation is paid in full at the first treatment visit.
B) Care Credit (for fees over $300):
With fast approval over the phone from Care Credit, your payments can be much lower thatn those available through our office. Care Credit specializes exclusively in helping patients with larger dental cases to do the treatment they want. Care Credit offers no interest plans and carries fixed rates between 5.9% and 12.9% and can extend terms out to 60 months. There is no prepayment penalty. We will assist you in contacting them from our office or you can visit www.carecredit.com.
C) Three Payments (for fees over $1,000):
Total patient obligation may be divided as follows: 50% due at the first treatment visit, with the remaining balance split into two equal payments, due 30 and 60 days after the first treatment visit. For any fees under $1,000, the full amount is due at the initiation of any procedure. Note: balance payments will be written at the initiation of treatment, post-dated for 30 and 60 days. Our guarantee: If a post-dated payment is deposited prior to the date on the face of the check (or credit card slip) we will credit your account for an amount equal and in addition to that payment.
D) Credit Card Pre-Authorization
You can choose to keep your signature on file for either a one-time authorization or for continuous authorization to cover any unpaid balance after the insurance payment for any treatment performed in this office.
FORMS of PAYMENT and BALANCES DUE
In order to facilitate access to the very best health care possible you may choose for any of the following(including any combination thereof): Cash, Visa, Mastercard, American Express, Discover, Money Order, Personal Checks, or Care Credit (see above). Balances over 60 days will incur a finance charge of 18% APR.
INSURANCE
It is our pleasure to assist you in maximizing your insurance benefit by completing your claim forms. If your carrier is up to date (in over 70% of the cases), the claims will be transmitted via computer modem before the end of the treatment day! As a courtesy, in addition to filing the claim, we will initially ask you only for your estimated co-payment. Please understand that this is only an estimate, and is based upon the information available to us.
The range of benefits depends solely on what your employer wishes to purchase. Some plans cover as little as 30% or as much as 100% of dental services, with most falling in the 40% to 80% range.
Some plans base the amount of benefit on a schedule of fees arbitrarily developed by insurance companies. For this reason, you may receive a lower percentage that the reimbursement level indicated in your dental plan. For example, if your plan states that it will pay 80% of the cost of a specific treatment, it means 80% of the fee arbitrarily determined by the insurance company and not the actual fee charged by our office. The financial obligationf or dental treatment is between you and our office. The insurance company is responsible to you, and not to our office. We will assist you in any way that we can (including our brand new high tech electronic claims submission). Once your carrier has paid the claim, any difference will be due upon receipt of our statement. If for any reason, we have not received your insurance carrier's payment 90 days after the claim was submitted, the remaining balance will be due and payable by you, and subject to 18% APR.

Why this program was developed:
Over the years, our office has tried to provide quality dental care at an affordable price; however, the cost of health and dental care continues to increase. In an effort to encourage good dental health and assist with rising dental costs, we have developed an ?in-house? dental program available to patients who cannot obtain any other type of coverage. Since regular dental insurance programs usually require group participation to get any price reduction, individuals and families seeking dental insurance on their own find it too expensive to even consider. Our dental program provides a wide scope of benefits at an affordable price to help minimize the cost of your dental needs.
